Hypertrophic scars(HSs) and keloids are fibroproliferative skin disorders arising from deep skin injuries. While both involve excessive scar tissue formation, keloids exhibit invasive growth beyond the original wound margins, with high recurrence rates and heterogeneous clinical manifestations. Our reseach reveals sustained WNT activation and adipogenic suppression in keloids, implicating adipocyte plasticity in pathological scarring. Clinically, We found that intralesional corticosteroid injections show limited efficacy, potentially due to epidermoid cysts, which may exacerbate inflammation and tension. Pre-treatment ultrasound evaluation is recommended to optimize therapy. Besides, our modified minimally invasive core excision demonstrated low recurrence and high satisfaction for joint keloids, while extended-interval postoperative superficial radiotherapy combined with surgery reduced total radiation dose without compromising efficacy. Also, we explored postoperative tension-reducing measures (e.g., sports bras) enhanced recovery, especially in large-breasted patients. Our work advocates multidimensional, personalized scar management combining targeted surgery, optimized adjuvant therapies, and biomechanical modulation to improve outcomes.
